Health, immunization, drug screen, fingerprinting and CPR requirements
The College of Nursing & Healthcare Innovation requires that students wishing to advance to the upper division Nursing major provide written and dated documentation of the items listed below.

Proof of Measles, Mumps, and Rubella immunity by either:
- Proof of two doses of the MMR (Measles, Mumps, Rubella) vaccine -- one of which must have been given after December 31, 1979; OR
- Proof of one dose of MMR and proof of immunity for Measles (Rubeola) and Rubella by submitting laboratory tests for positive titer levels; OR
- Proof of immunity for Measles (Rubeola), Mumps, and Rubella by submitting laboratory tests with positive titer levels.

Proof of Hepatitis B immunity by either:
- Proof of three doses of the Hepatitis B vaccine; OR
- Proof of immunity to Hepatitis B by submitting a laboratory test with a positive titer level.

Proof of a Tetanus/Diphtheria (Td) vaccine within the past ten (10) years.

Proof of a Varicella (Chicken Pox) by either:
- Proof of two doses of the Varicella vaccine; OR
- Proof of immunity to Varicella by submitting a laboratory test with a positive titer level.

Proof of a current negative TB (tuberculosis) skin test. If the skin test is positive, proof of follow-up treatment is required. A skin test (or follow up) is required every 12 months while students are participating in clinical courses.

Proof of current CPR certification at “Level C” (Health Care Provider). To be submitted if admitted to the Professional Nursing Program.

Proof of a negative drug screen. To be submitted if student secures a space in the upper division Nursing major. Specific testing information and approved laboratory collection sites will be provided.

Proof of submission of application for a fingerprint clearance card if student secures a space in the upper division Nursing major. Dates and location for fingerprinting will be provided by the Student Services Office.

Additional laboratory tests, immunizations, or documentation may be required. Students will be notified accordingly.

Statement of Health Insurance and Physical Exam by healthcare provider showing fitness to be a nursing student.
Note: Hepatitis A vaccination is recommended but not required.
The Student Services Office will accept only photocopies of the required documentation. Also, please note that copies are not retained by the Student Services Office following data entry into the students’ records; therefore, all students are required to maintain their own records of current documentation (i.e. TB skin tests, MMR, Hepatitis B, CPR certification, etc.).
