International Health for Health Professionals
Admission Requirements:
  • Baccalaureate prepared nurse or health related professional (e.g., social worker, physician, psychologist, dentist) with a bachelor’s, master’s, or medical degree from a regionally accepted university or equivalent.
  • ASU Graduate College requirements for admission as a non-degree seeking applicant must be met.
  • Professional license in the state where the Individual Capstone project will be conducted, if required (e.g., RN, NP, MSW, PA and MD).
  • Fingerprint clearance for U.S. Citizens.
  • A current Passport.
  • Resume or curriculum vitae.
  • Three letters of professional reference.
  • Only completed applications will be reviewed.
steps to apply:

1. Complete the Graduate College online application for this certificate. Upload your resume within this application, as requested. Remember to request one set of official transcripts from every college and university from which a Bachelors or higher degree was earned. Please request transcripts early as they may take weeks to arrive. Send official transcripts to:

2. Ask for three professional references to complete a Reference Request Form. For each one:

  • Fill in your name and address in the space provided.
  • Write your name at the top of each page in the space provided.
  • Give a copy of the Reference Request Form to each individual who has agreed to be your reference.
  • Request a deadline for mailing in the form.
  • Select your references from the following groups: Your most recent employer; A professor who can assess your academic performance and judgment in the area in which you wish to study; A professional person (not a relative by blood or marriage) who is well-acquainted with your professional work and academic achievement.
  • Provide each reference with a stamped envelope, addressed to:

3. Provide Fingerprint Clearance or Proof of Clearance - All applicants need to show proof of either current fingerprint clearance or application to obtain fingerprint clearance in the state in which they reside.  These fingerprint cards include a basic criminal background check.

4. Complete Applicant Questionnaire - Complete the Applicant Questionnaire in this packet. Questionnaire responses should be numbered and typed on a separate sheet of paper(s) and attached to the questionnaire. Sign and mail the questionnaire with your application materials (See step 6).

5. Complete the Applicant Self Assessment Form - Complete the Applicant Self Assessment Form in this packet. Write your name at the top of each page in the space provided. Sign and mail the completed Applicant Self Assessment with your application materials (See step 6).

6. Mail Application Materials - Mail the following materials to the address below:

  • Photocopy of fingerprint card or proof of application for fingerprint clearance
  • Completed and signed Applicant Questionnaire
  • Completed and signed Applicant Self Assessment Form and mail to:
