APPLICATION AND ADMISSIONS INFORMATION

The Graduate Certificate in Community and Public Health Practice is open to post Bachelor of Science individuals with experience or interest in working in community, public health, or related fields. Students must submit materials listed below and will be evaluated by faculty in the College of Nursing. The length of the program will be determined by the amount of community/public health experience. A minimum of 1 credit of internship is required by all and up to 3 credits of internship for those with minimal community/public health experience.

Admission Requirements:
  • A bachelors degree (in nursing or related field) and interest in increasing knowledge in Community/Public Health or
  • Current student in a Master of Science degree program or doctorate program at ASU.
  • International applicants must also meet the requirements of the ASU Graduate College.
  • All courses can be applied to a Masters in Community Health Nursing. However, students can only transfer 9 credits into the Masters programs at ASU; therefore, you need to submit an application to the MS degree program prior to completing 9 units for all credits (18) to count towards the Masters degree.
Materials Required for Application Process:

1. Apply to the Graduate College - Click “Apply Now,” then select “Create ID,” and click “Degree/Certificate Seeking Student” as the Application Type. In the "Degree/Certificate" Section later in the application, please select "Community and Public Health Practice Grad Certificate" found in the category "Health Sciences and Allied Fields."
2. Send official transcripts from all universities attended to: Graduate College, Arizona State University, Administration B-Wing, Room 170, PO Box 871003, Tempe, AZ 85287-1003.
3. Three professional letters of recommendation (MS Word or PDF) -
US mail to Pamela Kulikowski, College of Nursing & Healthcare Innovation, Arizona State University, 500 N. 3rd Street, Phoenix, AZ  85004-0698.
